GED Transcript/Duplicate Certificate Request Form

We ask that you please call our office to verify we have your GED on record before sending  in a request form. 

We only accept request forms by mail or if they are brought into our Harrisburg location.  We currently only accept payment by cash or money order. We cannot accept credit card payments or emailed request at this time.  Once we verify the record and receive the request with payment, we can then mail and email a copy.

Effective July 1, 2026, Illinois GED test-takers must use the code ILSAVE2026 to receive $26 off the cost of the GED test. The current code, ILSAVE26, will no longer be valid after June 30, 2026.
